Looking for a Targeted Case Manager for our Forrest City Office!

Responsibilities
- Complete client assessments
- Complete renewals
- Monitor and update client progress
- Referrals and Linkage
- Be familiar with Activities of Daily Living
- Report to the TCM Supervisor
- Have at least 1 year experience as LPN or 2 years experience in social work.
Must be licensed in the state of AR as a LPN or have 2 years case management experience. Wages are based on experience. Great demeanor and care for the elderly, children and disabled is a must! Must be trustworthy, have a clean background check and be drug free.
