Location BMH Bronson Methodist Hospital Title Clinical Care Supervisor: Pediatric Hematology/Oncology - Kalamazoo, MI Under general supervision from the Director of Practice Operations, and in accordance with the policies and guidelines established within the organization and practices, the Clinical Care Supervisor's priority is to function as leader and clinical liaison with staff, physicians and management, providing continuity of leadership day-to-day for the Practice Clinical Care Coordinators. The Clinical Care Supervisor acts as a role model, mentor, coach and resource person to staff to ensure all unit standards are maintained and to encourage a team environment. The Clinical Care Supervisor is involved in assisting managers in attracting, developing, maintaining, and retaining qualified staff members. Duties may include assisting with hiring, orientation, monitoring and evaluating performance, problem solving, employee counseling, staffing, payroll and budget. In addition to duties involving unit personnel, the Clinical Care Supervisor serves as a contact for internal and external customers. He/she is responsible for maintaining a safe environment of care, which fosters positive customer service and meets all quality standards, implementing new and monitoring ongoing processes. This role provides opportunity for leadership succession. The Clinical Care Supervisor provides oversight to the Practice Clinical Care Coordinators who are responsible for navigating patients through transitions of care in the healthcare system, inpatient services to outpatient services to home, based on individual and patient population needs. Serves as a clinical resource to patients, families, and staff in the delivery of care to all patients. Works collaboratively with the interdisciplinary team to provide a continuum of care that is efficient, cost-effective, minimizes fragmentation, and achieves desired treatment outcomes. Ensures communication between patient/family and all healthcare providers. Employees providing direct patient care must demonstrate competencies specific to the population served. Bachelor's degree in Nursing or equivalent combination of education and experience and 2-5 years of experience including clinical nursing experience in an acute care setting required Master's degree preferred Licensed RN in good standing with the State of Michigan required. Ability to utilize word processing, spreadsheet, presentation programs, databases, and other software releant to the job Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time. The job produces some physical demands. Typical of jobs that include regular walking, standing, stooping, bending, sitting, and some lifting of light weight objects. • Coordinates and supports care management activities and operations within the practices and the organization • Reviews current literature and provides recommendations for improvement activities • Evaluates organizational data and understands variation and trends • Leads the Practice Clinical Care Coordinator teams through change process • Participates in evaluating performance of staff within the department. Provides ongoing feedback to staff and practice manager related to job performance (focuses on immediate performance coaching as necessary). Monitors staff performance on a day-to-day basis. Works with manager to develop Personal Development Plans including follow up and mentoring. • Participates in discipline process of employees with manager notification for attendance, behavioral and performance issues. • Facilitates Practice Clinical Care Coordinator team meetings. • Participates in recruitment, selection and retention of personnel. Assists manager in the staff hiring process (e.g. assist with interview process). • Establishes specialized and specific inpatient and/or outpatient multidisciplinary managed care plans for high-risk patients. • Works collaboratively with the interdisciplinary team to provide a continuum of care, that is efficient, cost-effective, minimizes fragmentation, and achieves desired treatment outcomes. • Provides oversight of inpatient and/or outpatient plan of care to ensure coordination and completion of services to meet post hospitalization needs. • Acts as a clinical resource to patients, families, and staff in the delivery of care to all patients. • Acts as clinical expert resource, mentoring staff, maintains positive relationships, and builds teamwork within the entire healthcare team. • Acts as a liaison and facilitator between the patient, hospitalist, staff members, case managers, and the primary care physician. • Facilitates one on one communication with primary care physician regarding current and ongoing patient care needs. • Facilitates multi-disciplinary rounds for high-risk patients. • Follows up on discharged patients within 48 hours of discharge. • Triages patient/family calls after discharge. • Navigates patients through the healthcare system based on individual and patient population needs. • Identifies appropriate venue and resources for care within the continuum. • Track hospitalist program data as requested. • Utilizes critical thinking and problem solving techniques. • Is able to work independently, exercising sound judgment, discretion, and initiative to facilitate change. • Documents required work processes and procedures. • Records data correctly and in standardized format on computer and in writing. • Participates with and contributes to the development of strategic initiatives related to transitions of care. • Prepares patients for examinations, including patient assessment, histories, vital signs, medication reconciliation, and routine testing.
