The University of Miami Health System Department of UMHC - SCCC Site Disease has an exciting opportunity for a full time Patient Navigator to work at Syvester Cancer Center in Miami. The incumben facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers. CORE JOB FUNCTIONS Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan. Ensures patients have access to insurance and social service programs for which they are eligible. Provides patient reminders of upcoming appointments and acts as liaison between patients and providers. Identifies and resolves real or perceived barriers to care. Educates and counsels patients on issues related to their health and life situations. Maintains current knowledge of community health, welfare resources, and any other programs available to patients. Adheres to University and unit-level policies and procedures and safeguards University assets. Department Specific Functions Telephone responsibilities include, but are not limited to speaking to the patient, and contacting a variety of health care facilities to obtain patient medical record information. Address barriers and gaps in care with the nurse navigator. Follow-up on interventions from the nurse navigator to remove barriers (i.e. referrals-internal/external). Provides related internal, local, or national resources to the patient, family, or caregiver. Explains consent for obtaining outside records, and HIPAA (if applicable) to patient; obtains signatures (may be done via email) Submits signed consent to health care centers as appropriate to initiate retrieval of medical records, pathology slides, x-rays and any other pertinent report Tracks pending records to ensure delivery prior to clinic visit such as pathology reports etc. Communicates to the appropriate SDG Nurse Navigator, if difficult or unable to obtain a pertinent record Travels within the facility and campus to retrieve records/films/CD/s as needed Prepares pathology slides and submits for consultation Returns pathology slide to appropriate institution or patient Communicates with physician as needed Assembles, completes, and processes patients’ charts as indicated Maintains confidentiality of patient information. Maintains an accurate clinical record, recording information per hospital policy. Performs patient call back to complete Patient Satisfaction Survey. Reports unusual incidents in a timely manner to appropriate supervisory personnel. Assist nursing staff in collaboration of multidisciplinary meetings; including tumor conference coordination. Assists with other clerical duties, not limited to faxing, scanning, copying Performs other duties as assigned. Education: High School diploma or equivalent required Experience: Minimum 3 years of relevant work experience required Knowledge, Skills and Attitudes: Ability to process and handle confidential information with discretion. Ability to communicate effectively in both oral and written form. Ability to handle difficult and stressful situations with professional composure. Ability to maintain effective interpersonal relationships. Ability to understand and follow instructions.
