Building Location: Duluth Family Medicine Clinic Department: 39800 DFMC Clinic Operations Job Description: We are looking for someone with the gift of caring to work as a full-time Community Health Worker. CHWs are natural helpers and individuals who have a close understanding of the communities they serve. They are specifically hired and trained to address behavioral and social determinants of health through patient-centered goal setting. CHWs reach beyond clinic walls to help patients meet their goals. They creatively leverage patient, family, and community resources to address root causes of patient’s health concerns. CHWs work to increase access to medical, behavioral health, and community services. The goal of this team is to improve patient overall wellbeing through developing trusted relationships with the patient and their family. Education Qualifications: Required Education: None Preferred Education: Bachelors in healthcare, population health, community health, or a related field Community Health Worker (CHW) certificate Required Qualifications: 1 of experience in customer service, social service, healthcare, behavioral health, or a related field Preferred Qualifications: 2 years working in community or behavioral health Strong written and verbal communication skills Facilitation skills A desire to make a healthy difference in people's lives A long term interest in working in community health Duluth, MN – Like Nowhere Else!
