Licensed SW Maternal Child Health

SW Role:  They will assess social determinants of health by completing biopsychosocial assessments with the goal of increasing access to quality health care and reducing barriers to discharge.  They will provide patients and families with emotional and resource support by providing interventions that are strength-based, focused on the whole-person, trauma informed and span across the life cycle of our patients.  They will collaborate with the multi-disciplinary teams on how the illness or injury may impact the patient’s, family, employment and living situation. They will promote health and reduce health inequities by advocating for, educating and empowering patients and families.
