JOB SUMMARY:
Assumes responsibility for individual medical case management of agency clients, on a case-by-case basis. Responsible for ensuring the quality of medical care provided meets DDD and Agency standards while working cooperatively with the Director of Residential Services and the Executive Director to uphold agency and licensing regulations.
ESSENTIAL FUNCTIONS / DUTIES:
- Monitors the health of the individuals through review of Professional Medical Reports.
- Assesses the educational needs of the staff and conducts or arranges for appropriate in-service training programs.
- Reviews all client accident incident reports and provides medical input where indicated.
- Reports to a physical site to review all the in house medical records. Ensures the timely completion of medical follow up and annual examinations and provides recommendations for care. Records progress notes at program sites with copies forwarded to the site SPA and Director of Residential Services.
- Advocates in the best interest of the clients at all times, acting as a medical liaison between staff and hospital/medical professionals when necessary.
- Attends all mandatory Agency training and other training as is identified by the Director of Residential Services.
- Maintains the confidentiality, security and integrity for the management of client information according to agency and DDD policies
- Communicates and collaborates with other team members and community agencies in planning, implementing and coordination and evaluating all services necessary to insure comprehensive care.
- Cooperates fully with licensee and Department of Human Services staff in any inspection or investigation.
- Performs other duties as may be assigned by Director of Residential Services and/or the Executive Director.
QUALIFICATIONS/EDUCATION:
- Must be at least 18 years old, have a valid NJ Driver’s License with an acceptable record of safe driving
- High School Diploma / GED (verifiable)
PLUS
- Associates or Bachelor’s degree nursing.
- New Jersey Nursing license.
- 2 years’ experience in the nursing field.
- Ability to determine the needs and abilities of the individuals and solve problems systematically.
UPON EMPLOYMENT:
- Physical exam stating applicant can perform duties of the job.
- Proof of negative drug test results.
- Proof of Mantoux skin test showing negative results.

- Ability to pass a Child Abuse Record Information (CARI) check.
- Ability to pass a background check (fingerprinting, DHS Central Registry, Medicaid Fraud Exclusion List)
Job Type: Part-time
Pay: $37.30 - $38.30 per hour
