As a case management physical therapist with 21st Century Home Health Services Inc., you will be responsible for evaluating and treating patients in their homes, developing care plans, and supervising Physical Therapy Assistants (PTAs). You will work closely with an interdisciplinary team to provide comprehensive care, addressing the unique needs of each patient.
Your expertise will be valued as you help patients regain their independence, mobility, and movement. You will communicate effectively with patients' physicians to ensure a high-quality level of care, reporting progress and adapting treatment plans as needed.

To succeed in this role, you will need:
- A current unrestricted CA Physical Therapist License
- 1 year experience supervising PTAs in a HH environment (preferred)
- Current BLS/CPR Certification
- Valid CA Driver's License
- Knowledge of local community resources
- Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on healthcare
