What are the responsibilities and job description for the Social Worker/Community Engagement Specialist (Aged Population) position at 22nd Century Technologies Inc.?
Primary Responsibilities:
- Establishing awareness of Caregiver Portfolio, Caregiver Education, dementia care services and disseminating caregiver information
- Presenting trainings and workshops on caregiving and dementia as well as the coordination of outreach for Memory Cafes run by delegate agencies.
- Supporting presentations and programming at the Regional and Satellite Senior Centers and community partners such as aldermanic offices, hospitals, and libraries.
- Providing one to one caregiver support and caregiver program recruitment.
- Administering TCARE screens and assessments and providing SHIP information and Medicare / Medicaid counseling.
- Entering data information into Salesforce
- Community outreach
- Other duties as assigned
Qualifications:
- Graduation from an accredited college or university with a Bachelor’s degree or higher in Gerontology, Psychology, Social Services, Public Health or a directly related field preferred; or at least two years of work experience in the planning, implementation, and administration of social service programs and / or lived experience as a family caregiver.
- Previous experience an emphasis on Senior Services or the aging population and or family caregivers
- Knowledge of social services programs and resources
- Knowledge of resources and services provided by community social service agencies
