Job Overview
- Job Title: Physical Therapist
- Location: Eureka, California
- Type of Assignment: Contract (13 weeks)
- Start Date: 4/21/2025
- Shift: 5x8 Hours, 40 hours per week
- Shift Hours: 08:00 - 16:30
- Pay Rate: $58 - $60 per hour
Qualifications:
- Valid California PT License
- BLS Certification (must be through American Heart Association - AHA)
- Minimum 1-year recent experience as a Physical Therapist (PT) in a Skilled Home Health
- Position is for Home Health - Humboldt County CA and surrounding areas
- Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)
- System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50 miles
- 1 Weekend Shift required
- Candidate is expected to have their own transportation.
- Facility is open to 26 weeks contract
- Requires primary series COVID vaccination in addition to one booster
