The Field Specialist is a member of the home care team who works under the supervision of the Director of Nursing and performs various personal care services as necessary to meet the client’s needs. This position is managed by the Customer Care Manager. The Field Specialist will provide direct care for short-notice assignments. 

MAJOR DUTIES AND RESPONSIBILITIES 

  • Provide client care as directed by Customer Care Manager.
  • Provide personal care included by not limited to:
    1. Bathing and grooming, including oral care, assisting the client with dressing and undressing with toileting activities.
    2. Assisting in feeding the client, if necessary, including feeding clients who are unable to feed themselves.
    3. Taking and recording vital signs (temperature, pulse, respirations and blood pressure), measuring and recording height and weight, if necessary.
    4. Supporting good skin care to prevent skin breakdown (i.e. turning and repositioning client, actively encouraging movement) and assisting client with active and passive range of motions as defined by the plan of care.  Assists clients with walking and transferring if necessary.
    5. Observing client’s physical, mental and emotional conditions, documenting observed changes and reporting any changes to the Director of Nursing or Customer Care Manager. Measuring and documenting intake and output if necessary.
    6. Assisting the client in the self-administration of medication (medication reminder).
    7. Performing light housekeeping, meal preparation and clean up, changing bed linens, making up the client’s bed and maintaining an orderly client environment. 
    8. Develop and promote a safe environment for quality client care through adherence to established policies, procedures, and standards.
  • Availability and scheduling includes but is not limited to:
    1. Accepts a weekly schedule based on 5 days per week/8-hour shifts.
    2. Returns calls to the office in a prompt manner to accept new assignments.
    3. Provides care for new clients as they begin services.
    4. Fills in for call-ins and planned vacations for other caregivers.
    5. Available and willing to work with any type of client population including and not limited to children, adults, or seniors in a variety of health conditions and diagnoses.
    6. Open to all home environments that may include pets or smoking.
    7. Able to help in the office or with other administrative duties if needed (complete introductions, answer phones, organize and clean) if no client shifts are available.   
  • Training:
    1. Performs on-the-job training for newly hired caregivers.

REQUIRED QUALIFICATIONS 

EDUCATION

  • High school diploma or equivalent required.

SKILLS/ABILITIES/OTHER REQUIREMENTS

  • Certified CPR and current physical, or TB/PPD test, where applicable
  • Has reliable transportation that is insured in accordance with the organization’s requirements.
  • Willing to travel anywhere within the location’s service area.
  • Ability to adhere to HIPAA and maintain client confidentiality.
  • Ability to read, write, speak and understand English and communicate effectively.

WORKING CONDITIONS 

  • Travel required to cover the entire service area as needed for various client care service needs, as well as staff and business needs including attending job required training and conferences.
  • Contact with clients under a wide variety of circumstances. Care provided in client’s living environment with varying situations or in an institution such as a hospital or long-term care facility. 
  • May be required to respond to emergency situations. 
  • Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.
  • Personal Protective Equipment may include gloves, mask, eye/face protection, and disposable outer covering or lab coat.

 PHYSICAL REQUIREMENTS 

Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the ability of the client. Must be able to lift to 50 pounds. Visual and hearing acuity required.
