We're hiring a compassionate, self-motivated Physical Therapist to join a top-tier home health team serving patients across Lansing, Michigan. ️ Key Responsibilities- Provide skilled physical therapy treatment to home health patients
- Collaborate with interdisciplinary care teams to implement patient care plans
- Maintain accurate, timely clinical documentation in the EMR system
- Communicate with patients, families, and other healthcare providers
- Utilize company-provided tablet and smartphone to complete documentation efficiently
- Education: Bachelor's degree or higher in Physical Therapy from an accredited institution
- License: Valid Michigan Physical Therapist license
- Experience:
- 1 years of healthcare experience
- 1 years of experience with any of the following:
- Electronic Medical Records (EMR)
- TruChart EHR or other EMR systems
- Certifications: Valid Driver's License (DL)
- Must not have applied to or worked for Residential Home Health & Hospice in the past 6 months
- Must have spoken with a Relode recruiter about this position
