What are the responsibilities and job description for the Case Manager position at 24th Judicial District Court?
The 24th JDC Specialty Treatment Court Program is currently hiring a Case Manager to manage and report participant progress. Specialty Treatment Court is a criminal justice program that provides addiction and mental health treatment to offenders on probation. The program services men and women from age 18 and up. Case Manager duties include planning, implementing, monitoring and evaluating actions required to meet the participant services needed.
Responsibilities
- Coordinate and provide services that is safe, timely, effective, efficient, equitable, and participant-centered
- Handle case assignments, draft compliance plans, review case progress and determine service referrals based on need
- Help participants achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Develop effective working relations and cooperate with treatment, medical, family and legal team throughout the entire case management process
- Frequent interaction with participants to keep track of their progress and to ensure compliance
- Record case information, complete accurately all necessary forms and produce statistical reports
- Promote quality and cost-effective interventions and outcomes
- Assess and address motivational and psychosocial issues
- Adhere to professional standards as outlined by protocols, rules and regulations
- Weekly reporting on progress, infractions, needs, etc.
- Prepare recommendations for infraction response and progress incentives
- Prepare and present weekly status updates to team members
- Prepare and present weekly status updates in court before supervising judge
- Monitor and report drug screen results
- Record all interaction and participant progress in a state provided database
- Documentation of in person or virtual visits, phone calls, messages, drug screen results, referrals, treatment progress, court hearings and recommendations
- Team communication using SLACK or TEAMS platforms
- Prepare participants for graduation and facilitate alumni groups
- Maintain confidentiality
Skills
- Proven working experience in case management, medical, mental health care manager or a related job
- Excellent knowledge of case management principles

- BS degree in social work/counseling or related field, Master Degree preferred
Job Type: Full-time
Pay: $19.00 - $22.00 per hour
Expected hours: 35 per week
