Inspire health. Serve with compassion. Be the difference. Job Summary This position reports to the Outreach/Intake Manager RN and performs duties to support PACE Outreach and Intake activities. Answers incoming calls, provides education, takes messages, and forwards communications as appropriate. Independently handles a wide variety of PACE outreach and intake functions under the supervision of the Outreach/Intake Manager RN to include marketing and outreach activities. Interacts directly with potential PACE participants and their families for Prisma Health SeniorCare PACE. Supports Intake Department with recording and tracking potential referrals for the PACE program, data tracking, and with preparation of reports for Executive Leadership regarding PACE outreach and intake activities. Supports department with recording and tracking potential referrals, tracks data, prepares reports regarding department activities. Answers the Intake phone lines and assists caller appropriately. Assists with scheduling intake assessments and marketing and educational opportunities. Provides assistance to potential participants/families with the Medicaid application process and supports the department with coordinating community resources for potential participants Supports department with community outreach (i.e. preparation of materials, presentations, logistics). Performs other duties as assigned. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. Minimum Requirements Education - Associate degree in related field of study.
