POSITION STATUS: Non-Exempt
PAY STRUCTURE Salary Range (Min $16.00-$17.50 hour)*based on experience and education.

General Summary:
The Certified Nursing Assistant - Direct Support Professional (I) is responsible for providing compassionate care and support to individuals with intellectual and developmental disabilities. Responsibilities include assisting individuals with personal hygiene, feeding, administering medications, transporting to/from the day center and community outings, and maintaining detailed documentation of all care activities throughout the shift.
*Following successful completion of 360 Community Services’ advanced training program, CNA- Direct Support Professional (I) will be promoted to CNA- Direct Support Professional (II) status, which includes a $1.00 increase on top of their base hiring wage.
Principle Duties and Responsibilities:
- Ensuring all clients are treated with respect and dignity, recognizing individual needs and encouraging independence. Develop a rapport with clients and cultivate a safe and supportive relationship
- Provide supervision and guidance for clients during habilitation and community integration activities. Ensure an appropriate staff-to-client ratio, and always maintain the safety and security of the clients through line-of-sight supervision
- Responsible in observing your client’s negative behavior, the duration, the intensity, and the damage, factors that may alleviate or aggravate the behavior
- Assist with and work to strengthen the individuals’ many daily living needs including but not limited to nutrition, hygiene, exercise, communication, and socialization skills
- Implements and documents progress on the clients individualized service programs (ISP), including habilitation, behavioral, and medical programs/protocols.
- Assist all participants in being actively involved in the daily activities to achieve greater independence using person-centered planning and self-determination.
- Update the client electronic record daily, which includes Habilitation and BSP programs, MAR (medication administration record), medical & safety protocols, incident reporting, and daily communication notes which are sent home to the parents/guardians
- Oversee arrivals and departures of participants and document attendance times in the client electronic record.
- Provides AM or PM transportation to clients to/from the Day Center. Ensures the safe loading/unloading and timely delivery of clients to their outings/appointments as well as our established policies and procedures. Drives Company vehicle in a safe and courteous manner in compliance with all traffic laws, safety guidelines, current federal, state and local standards and regulations. Ensures all seat belts and wheelchairs are fastened prior to moving vehicle.
- Participates in staff training and attends staff meetings.

Qualifications:
Education:
- High school diploma/GED required
- CNA certification - Preferred
Requirements:
- Minimum age of 19 years
- Valid Driver’s License
- Current vehicle Insurance upon request
- The following certifications are required -will provide train if necessary:
- CPR/1st Aid
- Medication Aide
- MANDT
Must be able to pass the following background checks:
- Nebraska Department of Health and Human Services Child & Adult Protective Services Central
- Criminal background check
- National Sex Offender Registry
SPECIAL SKILLS, ABILITIES AND EXPERIENCE:
Experience:
- Three years’ experience working with people with intellectual or other developmental disabilities preferred
- Management of stressful situations responsibly and effectively
- Provides input and assistance in planning and evaluating programs and clients’ goals.
- Maintenance and documentation of records of client’s accidents/incidents as required.
- Assistance of clients in being actively involved in the daily activities to achieve greater independence using person-centered planning and self-determination- ADL’s- activities of daily living.

Physical Requirements and Work Environment:
Physical Requirements:
- Utilize approved behavior management techniques including use of de-escalation room and passive physical restraint when de-escalation and/or physical intervention is necessary for crisis management
- Employee must be physically capable of transferring, assisting with transfers, assisting with lifting clients who require physical assistance.
- May be exposed to infectious/communicable diseases, blood borne pathogens and/or distraught individuals.
- Employee must be able to reach and bend as they assist clients with physical activities.
- Frequently required to stand and walk
- Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
Working Conditions:
- Both indoors and outdoors with varying temperatures.
- The noise level in the work environment is usually moderate to loud.
