Outpatient Case Manager

Outpatient Case Manager RN or LVNNorthridge, CA

360 is looking for an experienced Outpatient Case Manager in Northridge, CA! Ideal candidates will have at least 1 year of recent experience as an Outpatient Case Manager.

Pay Details:

• Pay Range: $37.00 - $50.00 hourly (depends on licensure and experience)

Work Setting:

• Hybrid (both in-office and work from home)

Position Details:

• Location: Northridge, CA
• Mon to Fri / 8:30am to 5pm or 8am to 5pm

Requirements:

• Requires a valid, unrestricted California RN or LVN license.
• Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
• Current CA RN, or LVN license, current CPR certification.
• 3 years acute care or case management experience preferred.
• 2-3 years of utilization or HMO experience preferred.
• Typing 40 words per minutes with accuracy.
• Knowledge of computers, faxes, printers and all other office equipment.
• Knowledgeable in MS Office Programs (i.e., Word, Excel, Outlook, Access and PowerPoint)
