What are the responsibilities and job description for the Housing Support Specialist (Scattered Sites) position at 3KEYS INC?
- Develop and maintain working relationship with City of Atlanta Coordinated Entry/Partners for Home and provide up to date vacancies in program
- Cultivate relationships with landlords willing to provide housing for participants
- Work with landlords via master lease to ensure positive relationships between program and property
- Review with each participant issues of renter expectations, renter landlord conflict resolution and tenant rights
- Receive 30% contributions as required from participants and prepare documentation for deposits
- Assist clients in maintaining housing, applying for benefits and assisting with accessing and getting furnishings for each unit
- Provide case management to a minimum of 30 clients
- Meet with residents a minimum of twice a month
- Provide independent living skills training and support, as needed
- Assist in making referrals to other community resources as needed
- Coordinate services and link participants to services in the community that will meet identified needs, as needed
- Develop and sustain relationship with clinical service providers, who will provide intensive case management for individuals, including debriefings about client needs and crisis management as needed
- Provide flexible services during times that meet needs of residents
- Maintain individual files, including case notes, intake and assessment information, homeless verification, disability verifications, identification, changes in income and other items as noted by program director.
- Complete all required Client Track HMIS information including entry and exit documentation, updated income information and weekly case notes and services provided
- Submit monthly reports based on services in the program
- Maintain accurate records
- Participate with funding reports, grant writing and site visits as requested
- Network with providers of similar services
- Participate in professional meetings, workshops, trainings and staff meetings
Position Requirements:
- Bachelors Degree in behavioral science, with a minimum of 5 years’ experience working with chronically homeless populations.
- Knowledge of local community resources
- Experience in landlord outreach
- Knowledge of HUD PSH programs, requirements and operations
- Working knowledge and experience with HMIS
- Reliable transportation
- Ability to follow oral and written instructions and work independently with minimum supervision
- Effective organizational and time management skills
