Adventist HealthCare seeks to hire an experienced Manager of the Advanced Primary Care Program who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.
As a Manager of the Advanced Primary Care Program, you will:
• Manage the administration of the Medicare value-based care programs, ensuring all regulatory requirements are met including, but not limited to financial reporting and management, patient survey administrations, roster updates, patient family advisory council, quality reporting, and participation agreements.
• Stay current with any program changes to Medicare value-based care programs, attend all program meetings, and serve as the liaison between the organization and the program(s).
• Support the administration of other value-based care programs for the CIN as needed, including quality measure and target selections, updating quality measure definitions, and supplemental file submissions.
• Regularly monitor and be accountable for quality, utilization, and financial performance of each program at a program level as well as at a practice level, and provide appropriate feedback to and/or determine whether additional support for each practice is needed.
• Oversee partnered primary care practices’ engagement for each value-based care program that they are a part of, ensuring each practice is sufficiently informed and engaged for the success of each program.
• Develop and improve upon advanced primary care programs to produce target performance results.
• Supervise and manage a team of Care Transformation Specialist(s) and Program Coordinator.
• Manage and develop monthly/bi-monthly meeting templates for practices in collaboration with the Care Transformation Specialist(s) and meet with primary care practices on a regular basis.
• Engage effectively with primary care practices and develop a good understanding of their workflows and potential opportunities for improvement.
• Support deployment of new value-based care IT solution(s) to primary care practices in collaboration with the Care Transformation Specialist(s).
• Serve as an administrative lead for the Clinical Committee and the Medicare value-based care program Boards.
• Participate and present relevant updates to the Commercial Council.
• Function as a subject matter expert of the population health platform.
• Serve as a subject matter expert for advanced primary care and support the health system as needed for population health programs.
• Identify data gaps or errors and support resolution of these issues through coordination with external vendors and/or other AHC departments, including the Physician Enterprise Business Analytics unit.
Qualifications include:
• Bachelor’ degree required with Master’s degree in analytical healthcare-related field preferred.
• 5 years of progressive healthcare-related project management and value-based care experience required.
• Population Health experience preferred.
• Previous supervisory experience required.
• Direct experience in Primary Care settings preferred.
• Light physical effort (lift/carry up to 10 lbs.).
• Occasional bending, lifting, stooping, kneeling, crouching.
• Some traveling to/from primary care practices.

Pay Range:
$83,112.12 - $124,675.20In cases where the range is displayed as a $0 amount or if the position is Per Diem (offering a fixed rate), salary discussions will occur during the screening process.
