Treatment is provided in the context of a multidisciplinary clinical team, supervised by a physician. Leadership and/or supervisory duties as delegated by manager. STATUS: Full Time DEPARTMENT: Women's Health and Wellness LOCATION: Pittsford Women's Health and Wellness/Hybrid SCHEDULE: Monday - Friday, 8AM - 4PM ATTRIBUTES Masters in Social Work Minimum of 3 years’ experience providing clinical social work services Licensed Clinical Social Worker in NYS (LCSW or LCSW-R) RESPONSIBILITIES Provides initial and ongoing assessment, Evidence-Based clinical interventions to a designated caseload. Develops Treatment Plans that are individualized and culturally sensitive Provides individual, group, and family psychotherapy. Participates functionally as a member of a multidisciplinary team in coordination and delivery of patient care. Communicates in a timely and complete manner with internal and external care providers to complete ongoing assessment and to coordinate care. Participates in and incorporates professional supervision and ongoing education toward quality care. Utilizes efficient computer skills for electronic documentation. Completes all required documentation in a timely and comprehensive manner. Maintains a caseload, meeting productivity requirements as stated by the Department of Behavioral Health, and seeks to keep the incidence of failed appointments to a minimum. Must maintain current licensure as a Licensed Clinical Social Worker (LCSW or LCSW-R) in NYS. Undertakes planning, problem solving and quality improvement activities regarding program functioning. Pursues personal and professional growth using internal and external sources available for supervision/consultation and continuing education. Provides clinical supervision/consultation/instruction to therapist interns and/or less experienced staff (LMSW or other) Assists Program Coordinator/Manager/Supervisors in assuring smooth, effective delivery of care in response to community requests for services. Credentialing to provide professional billing for clients who’s insurer allows for that provision In Outpatient Social Work: May include medical social work tasks of crisis intervention, social work assessments, managing referrals regarding psychosocial needs. In Inpatient Social Work: May include medical social work tasks of social work assessments, care planning, referrals for community services, discharge planning, and facilitating family conferences/case conferences. Other duties as assigned. EDUCATION: MS (Required) LICENSES / CERTIFICATIONS: LCSW - Licensed Clinical Social Worker - New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. PAY RANGE: $60,500.00 - $80,800.00 CITY: Rochester POSTAL CODE: 14618 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
