Case Manager - Neighborhood Foster Home
Do you want a meaningful career advocating for Hope, Homes & Healing for children in foster care? 4KIDS is looking for a Case Manager to provide overall support and ensure that tangible needs are met to support our enhanced neighborhood foster home located in Boca Raton caring for children ages 12-17.
Job Duties:
- Provide one-to-one supervision and intervention during therapeutic activities
- Provide skills training in accordance with the treatment plan
- Provide assistance to the family, caregivers, and others involved in implementing the treatment plan
- Provide therapeutic behavioral support on-site in primarily one-to-one interactions
- Provide support in implementing TBRI principles
- Connect families with external resources
- Assist parents with implementing schedules for school, school assignments, after school activities, medical, or other appointments
- Assist in managing contract requirements, including maintaining files, documenting visits, accurate reporting, etc.
- Maintains required documentation in the resident's files and Client Resource Records, including Individual Service Plan (ISP), progress reports, and other documents in compliance with state regulations
- Assists the Foster Care Supervisor in the interview, assessment, and intake process for potential residents using established guidelines and by working within established timeframes
- Applies discernment in conducting, reviewing, and giving input on initial psychosocial assessments on each new resident in order to determine strengths and areas of needed growth
- Ensures all residents are up-to-date on medical and dental appointments and communicates this information to ChildNet in a timely manner
- Works in collaboration with the Foster Care Supervisor, Childnet, and other community service organizations to ensure that each resident in the home receives appropriate services to meet his/her needs
- Assists in incident reporting, including follow-up and timely submission of reports
- Uses discernment and critical thinking skills to coach and support House Parents as needed
- Attends internal/external trainings to increase knowledge to support House Parents and children
Requirements:
- Bachelor's degree in social work or a human services field
- Two or more years of experience working with children ages 12-17
- Certification as Registered Behavior Technician (RBT) preferred
- Detailed knowledge and experience in case management and documentation
- Proficiency in Microsoft
- Strong interpersonal skills
- Strong written and verbal communication skills
- Ability to interview residents and make quality assessments
- Ability to counsel children and parents
- Maintain current certifications determined by program, including Child Welfare certified through the Florida Certification Board
- Valid Florida driver's license
