Job Summary
Enter preventive, disease-specific, and outcomes clinical measures from the patient record into the appropriate registry/database. Must become knowledgeable in ACO, HEDIS, and/or PQRS metrics, chart/data abstraction process and data entry process. Must be very accurate and attentive to detail because the data is used for public reporting, pay for performance and physician credentialing. Perform inter-rater reliability checks among abstractor peers. The data must be completed timely and have quality control checks. This position reports to the Manager of Clinical Data Governance. Must have above average communication skills, professional bearing and appearance. Must be willing to work on-site within the system and travel to non-WellStar provider practices.

Required Minimum Education
Graduation from an accredited school of nursing, or other healthcare related fields such as Pharmacy, Respiratory therapy, Psychology: Behavioral Health. Required
Required Minimum License(s) and Certification(s):All certifications are required upon hire unless otherwise stated.

Required Minimum Experience:
Minimum 2 years experience in an acute care nursing unit, i.e. ICU, CCU, Med surgery area, Behavioral Health, or in your area of healthcare work.

Mon-Fri 8:00am-5:00pm no less than 32hrs some flexibility in hours may be required; virtual and on-site visits to the community.
Graduation from accredited school of nursing, OR other healthcare related fields such as Pharmacy, Respiratory therapy, Psychology/Behavior Health, Social Work.
Minimum of 2 years experience in an acute care medical unit, i.e. ICU, CCU, Med Surgery area, Behavior Health, or in other area of healthcare work.
Basic computer. Microsoft skills. Ability to read and understand the medical record and enter data into database. Must have good communication skills.

Core Responsibilities And Essential Functions
Attends required educational sessions regarding:
- Annual updates in metrics
- Abstraction tool(s)
- View Only access to disparate medical records/paper records
- Serves as metric expert
- Keeps up with new specifications and changes.
- Reviews charts and abstracts required data element into data base.
- Researches specifications and or other appropriate support groups when uncertain if documentation is acceptable
- Keeps accurate list of cases abstracted and completed
- Meet all data submission deadlines
- Travel to physician offices as needed to complete office-based abstraction (non-WellStar systems)
- Must have own transportation to and from offices
- Schedule abstraction time with offices at the convenience of the provider
- Ensure that office personnel or provider is available to assist with EMR, paper record navigation
- Runs error reports after data entry and corrects all errors.
- Reviews validation reports as requested
- Any questions or concerns regarding diagnosis codes discuss and review with physician offices
- Communicate with CMS regarding questions, issues with the WEB tools and bring to the attention of your manager.
