We are seeking a Patient Referral Specialist & Marketer to join our team! This role is perfect for an RN, LPN, or RRT with extensive healthcare experience who is passionate about building relationships, driving referrals, and expanding our reach in the community.

At 5 Star Cares, we are seeking a Clinical Liaison aka- Patient Referral Specialist and Marketer to join our team. This position is ideal for Licensed Practical Nurses (LPNs), Registered Nurses (RNs), or Registered Respiratory Therapists (RRTs) with EXTENSIVE medical and healthcare experience who are passionate about patient advocacy and building relationships within the community.

Job Description :

The Patient Referral Specialist and Marketer will play a crucial role in expanding our pediatric and young adult clients base and ensuring seamless transitions for new patients into our care. This role involves both marketing activities to promote our services and the management of patient referrals to ensure a smooth onboarding process. This role is geared toward Pediatric patients and Medicaid Recipients.

Key Responsibilities

Marketing and Community Outreach :

Actively market and promote healthcare services to potential referral sources, including hospitals, clinics, and physicians.
Develop and maintain strong professional relationships with healthcare providers and community organizations.
Identify and secure new patient referrals while ensuring continuity of care.
Provide accurate and thorough information to patients and families about available services.
Represent the company in a professional manner at networking events, meetings, and community outreach programs.
Assist with the development and implementation of marketing and sales strategies to promote our services with knowledge of pediatric clients and Medicaid recipients.
Organize and participate in community events, health fairs, and other marketing activities as needed.
Create marketing materials such as brochures, flyers, and social media content.

Patient Referral Management

Have ESTABLISHED working relations in the field, receive and process patient referrals from various sources.

Clinical background, payer knowledge especially within public assistance is vital, also problem-solving skills, youll work with physicians, discharge planners and patient's families to plan the best continuum of care.

Communicate with referral sources to gather necessary patient information, clinical diagnosis and ensure a smooth referral process.

Coordinate with the clinical team to schedule initial patient assessments (aka meet-and-greet) and services.

Performs clinical assessments of referred clients, then counsels with client's family members to assist them in making the best care decisions

Partners with hospital case managers to create smooth patient transfer processes and help remove payer-related and operational barriers to getting appropriate patients into this highly effective level of care.

Maintain accurate records of referrals and follow up with referral sources as needed.

Customer Service

Serve as the primary point of contact for referral sources and new clients, remaining professional to ensure that quality of standard we have here at 5 Star Cares

Provide excellent customer service and address any concerns or questions from clients and family members and referral sources.

Ensure overall client satisfaction by facilitating smooth transitions into home health care services.

Collaboration

Work closely with the clinical team to understand patient needs and coordinate care.

Collaborate with the management team to develop and refine marketing strategies.

Provide feedback and insights from referral sources to improve service delivery and patient care.

Requirements

Associate of Arts in Science, Bachelor's degree in Marketing, Healthcare Administration or Business Marketing Development in Healthcare or a related field.

MUST have clinical experience in one of the following credentials : Registered Respiratory Therapist, Registered Nurse, Licensed Practical Nurse with marketing experience and knowledge of low-income programs and resources.

Ability to work independently and manage a flexible schedule.

Flexible, field-based role with a supportive team environment.

Strong understanding of home health care services and the healthcare industry.

Excellent communication and interpersonal skills.

Ability to build and maintain relationships with referral sources and community partners.

Strong organizational skills and attention to detail.

Proficiency in Microsoft Office Suite, various EMRs and CRM software.
