Position Overview: Clinical Coordinator – Group Home Setting
The Clinical Coordinator in a group home setting plays a pivotal role in overseeing the clinical care and services provided to consumers. This position is integral to ensuring the delivery of high-quality care through effective management of clinical staff, coordination of care plans, and strict adherence to healthcare regulations and standards.
Key Responsibilities:
- Clinical Care Management:
- Oversee the development, implementation, and evaluation of individualized care plans for residents.
- Ensure that all clinical care is delivered according to established protocols and standards.
- Monitor residents' health status and coordinate necessary medical interventions.
- Taking new orders and new appointments
- Inputting, maintaining and following up on documents related to appointments
- Monthly nursing notes/documents
- Resident Advocacy and Support:
- Communicate effectively with residents and their families about care plans, health status, and any changes in care.
- Will be responsible for 30 consumers, reports, orders, appointments and documentations
- Provide and update scheduling
- Setting up transportation for appointments and riding with in transport van.
Collaboration and Coordination:
- Work closely with external health care providers, including physicians, therapists, and pharmacists, to coordinate comprehensive care for consumers
- Accompanying consumers on appointments and being an advocate
- Medication orders and management
Quality Improvement:
- Implement and oversee quality improvement initiatives to enhance the care and services provided.
- Analyze clinical data and outcomes to identify areas for improvement.
- Develop and implement action plans to address identified issues and improve resident care.*MAY BE REQUIRED TO PERFORM DUTIES OUTSIDE OF THE DUTIES DESCRIBED*
Qualifications:
- Education: Associate or Bachelor’s degree in Nursing, Healthcare Administration, or a related field.
- Licensure: Current and valid RN license or LPN
- Experience: Minimum of 3-5 years of clinical experience, with at least 2 years in a healthcare admin or coordinator role.
- Experience in a doctor's office, community healthcare or group home or similar setting is highly preferred
Skills:
- MUST BE ORGANIZED AND COMPUTER SAVVY
- Strong clinical knowledge and expertise.
- Excellent leadership and management abilities.
- Exceptional communication and interpersonal skills.
- Ability to work collaboratively with a multidisciplinary team.
- Proficiency in electronic health records (EHR) and other clinical software.
- Strong organizational and problem-solving skills.
