Location: 5 Star Cares – Group Home Provider
Job Type: Part-time
Pay: $18.00 - $21.00 per hour
Schedule: Monday to Friday, with weekend availability as needed-flexible scheduling

Qualifications
- Current QIDP certification (formerly QSP/QMRP)
- Minimum 1 year of experience working with individuals with intellectual/developmental disabilities
- Experience in writing program plans, case notes, or other documentation related to implementation plans
- Must successfully complete pre-employment background and drug screenings
- Valid driver’s license meeting company-approved driver standards
- Basic computer proficiency
- Bachelor’s Degree in Human Services or a related field
Essential Functions
- Oversee and coordinate individual program plans, ensuring they align with each person’s needs and goals
- Foster collaborative communication with all disciplines and team members
- Ensure compliance with regulatory requirements for program plans and documentation
- Train and support staff in implementing individualized training, support, and behavior programs
- Monitor program effectiveness and make necessary adjustments for optimal outcomes
- Perform other duties as assigned
