Department: 12801 Advocate Aurora Health Corporate - Trauma Recovery Center: Condell Hospital Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: Bilingual Spanish Speaking Highly Preferred Monday- Friday 8 AM-4:30 PM, with occasional need of extended hours till 5:30p. Major Responsibilities: Supportive Services: Provides direct and indirect services to patients with emphasis on resource development, entitlements advocacy, outreach, and coordination of care. 1)Triage requests for crisis services; provides brief, supportive crisis intervention services with support from other team members. 2)Maintains periodic contact with patients by phone and by letter. 3)Assists patients who need resources and referral to other support services. 4)Communicates with other agency programs and/or human service agencies to obtain relevant patient information. 5)Administers screenings as necessary to patients. 6)Responsible for scheduling of patient services. 7)Provide assistance to clients for active involvement in the program, including completion of any necessary paperwork, arrangement for transportation and other services as needed to achieve positive outcomes while they are receiving TRC services and to help stabilize the client once services are complete. 8)Provide special assistance to those clients that are not comfortable receiving help directly from the criminal justice system by assisting the client with completing and filing the necessary legal paperwork. Responsible for the development of a volunteer peer support service which will include identifying and interviewing potential volunteers, engaging and training of these volunteers, coordination of peer activities and groups to support the empowerment and engagement of the volunteers and participants. 1)Develop activities to best meet the needs of the participants through individual support services such as a peer support resource, supportive engagement efforts and facilitation of support groups. 2)Responsible for the training of volunteers on core relevant topics such as coping skills, appropriate empathy, boundaries, identifying signs of second-hand trauma, and resources for participants. Performance of reception duties in a courteous and efficient manner. 1)Greets and directs patients and visitors, according to policy, creating a positive and competent first impression. 2)Answers phones and directs callers as appropriate serving as a resource for the Trauma Recovery Center. 3)Admits patients to all outpatient services completing appropriate HIPAA, consent forms and inputs admission information to database. 4)Labels all charts and documents. 5)Oversees banding process. Creates ID Bands on patients and is responsible for collection of bands at end of treatment day. 6)Orders any screenings as necessary. 7)Additional duties as assigned. Agency and Training Activities: Actively participates in agency activities in ways that make a positive contribution to agency operations. 1)Performs a quarterly utilization review on clinic charts as part of the quality assurance plan. 2)Identifies professional goals and enhances job-related skills through staff development activities. 3)Remains current on trends in trauma, behavioral health, and case management theory and practice. 4)Demonstrates constructive involvement with professionals in the agency and hospital community. 5)Participates in quality assurance, accreditation, and Continuous Quality Improvement (CQI) activities and other special projects. 6)Responsible for developing and maintaining positive working relationships among TRC leadership and staff, Medical Center personnel, physicians, and Community Partners in order to ensure goal attainment and outcomes. Participates on various teams and committees as requested. Performance of secretarial and administrative services in support of the department in a courteous efficient manner. 1)Coordinates individual meetings and team meetings requiring minimal direction. Organize special events: secure space, prepare memo and /or notify participants, arrange for equipment as necessary, assure room set-up, order refreshments, etc. Relays phone and verbal messages to the appropriate health provider, including physicians, psychologists and social workers. 2)Creates and types letters, memoranda, reports, meeting agendas, and other types of correspondence from rough drafts or provided copy. 3)Reviews, sorts, and responds to correspondence in collaboration with the appropriate leader. 4)Orders supplies, processes invoices and purchase orders including capital requests. 5)Maintains department files, forms, and records in compliance with Advocate Document Control Standards. 6)Handles department charges. 7)Copies, prepares and disperses documents as requested. 8)Creates and maintains orientation/admission packets for new patients. Education/Experience Required: High school diploma or GED. At least three years of healthcare experience. Demonstrated ability to work constructively with clients, treatment resources and the community. Knowledge, Skills & Abilities Required: Care management skills Courteous and professional Demonstrates keyboard skills and familiarity with a Windows environment Interpersonal communication skills Crisis Prevention Intervention Physical Requirements and Working Conditions: Ability to demonstrate a commitment to patient focused care Ability to work flexibly in a fast paced environment with established time constraints and in difficult situations May be exposed to patients that potentially could be violent Familiarity with the roles and functioning of other providers in the psychiatric services, mental health and substance abuse treatment networks.
