Home Healthcare Certified Nursing Assistant (CNA) Hospice Experience Required
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) with experience in hospice care. In this role, you will provide hands-on, patient-centered care for individuals facing terminal illnesses in the comfort of their own homes. Your experience in hospice care will help provide our patients with dignity, comfort, and the highest quality of life during their final stages.
Key Responsibilities :
- Provide personal care services to hospice patients in their homes, including bathing, dressing, grooming, and assisting with activities of daily living (ADLs).
- Assist with mobility by helping patients with transfers, walking, and repositioning to prevent bedsores and maintain comfort.
- Monitor patients' condition by tracking vital signs (e.g., temperature, pulse, and blood pressure) and observing changes in physical or mental status.
- Assist in the emotional and social well-being of patients, providing companionship and listening support as they navigate their end-of-life journey.
- Report any changes in patient conditions or needs to the supervising nurse or healthcare team to ensure timely and appropriate interventions.
- Support the hospice care plan by providing ongoing care as directed, including assisting with feeding, oral care, and respiratory support.
- Provide respite care for family members, allowing them time to rest while ensuring that their loved ones receive consistent, compassionate care.
- Assist with light housekeeping and ensuring that patients' living spaces are comfortable and clean.
- Educate family members on how to support the patient at home, addressing their care needs and providing tips for safe, effective caregiving.
- Ensure a safe environment for patients by assisting with fall prevention, proper use of medical equipment, and maintaining cleanliness.
