Home Health Speech Therapist (SLP)
We are looking for a compassionate and skilled Home Health Speech Therapist (SLP) to join in providing high-quality, patient-centered therapy services in the home setting. The Speech Therapist will assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders in patients, helping them regain their independence and improve their quality of life. You will work closely with a team of healthcare professionals to develop and implement individualized care plans for patients in the comfort of their own homes.
Key Responsibilities :
- Patient Assessment & Evaluation :
Conduct comprehensive assessments of patients speech, language, swallowing, and cognitive skills.
Provide direct speech-language therapy services to patients in their homes, addressing a wide range of conditions including speech disorders, language delays, cognitive impairments, and swallowing difficulties.
Collaborate with the interdisciplinary care team, including physicians, nurses, and other therapists, to ensure comprehensive care and treatment for the patient.
Educate patients and families on techniques to enhance communication and swallowing abilities, and how to manage any challenges at home.
Maintain accurate and up-to-date patient records, documenting assessments, interventions, treatment plans, and patient progress in compliance with regulatory and agency standards.
Stay current on best practices, emerging research, and new treatment techniques in speech therapy, particularly in home health settings.
Qualifications :
Current, active, and unencumbered Speech-Language Pathologist (SLP) license in the state of Indiana.
Minimum of 5 years of experience as a Speech Therapist, preferably with experience in home health or a similar setting.
Strong clinical assessment and treatment planning skills.
Compassionate, patient, and empathetic, with a passion for helping others improve their quality of life.
