Position Summary
7TH & Madison Extensions of Empowerment is seeking a compassionate, resourceful, and mission-driven Housing Support Coordinator to support individuals transitioning into and maintaining permanent housing. The ideal candidate will have a passion for serving underserved populations and will provide both pre-tenancy and tenancy-sustaining services using a Housing First approach.
Key Responsibilities:
General Duties:
- Build and maintain relationships with landlords
- Conduct home visits and provide case management to participants in permanent housing
- Assist individuals in finding and securing housing
- Identify housing funding sources and complete necessary applications
- Deliver housing-based education and credit counseling
- Link individuals to community resources, including mental and physical health services
- Help gather essential documentation and increase income opportunities
- Ensure participants are connected to services that enhance quality of life
Pre-Tenancy Services:
- Conduct housing-first assessments to identify strengths and obstacles
- Develop and monitor goal-oriented service plans
- Provide weekly (or more frequent) case management to support housing applications
- Assist with obtaining documentation required for housing applications
- Link clients with relevant services (mental health, substance use, etc.)
- Support clients in searching for affordable housing and navigating leasing
- Recruit landlords, work with housing authorities, and support move-ins
Tenancy Sustaining Services:
- Continue case management after housing placement
- Conduct regular home visits to support housing retention
- Assist with adjustment to independent living and address transitional challenges
- Provide monthly budgeting and credit repair sessions
- Support clients through annual recertification and income sustainability
- Help maintain benefits such as SNAP to prevent food insecurity
- Collaborate with mental health/substance abuse providers for coordinated care
- Participate in program and case management meetings
Required Qualifications:
- Must hold a Bachelor’s degree in a related field and have at least 1 year of experience providing housing navigation or case management services to vulnerable populations. Knowledge of supportive housing, tenancy services, or Medicaid-funded programs is a plus.
- Experience with underserved populations, including mental health and substance use
- Strong communication skills and cultural competence
- Proficiency in Google Suite, Microsoft Office, and HMIS
- Strong organizational and time management skills
- Ability to manage multiple projects and meet deadlines
- Motivational Interviewing and Trauma-Informed Care experience (plus)
- SOAR or crisis de-escalation certification (plus)
- Bilingual (English/Spanish) (plus)
- Valid driver’s license with clean record
- Ability to travel 50% (mileage reimbursement or company vehicle provided)
- Ability to lift up to 35 lbs
Record keeping & Compliance:
- Maintain thorough and accurate case files and data in Salesforce, HMIS, and internal systems
- Record intakes, service plans, and case notes in a timely manner
- Compile monthly reports and track client progress
Meetings & Representation:
- Attend meetings in Essex County as needed
- Participate in case conferences, staff meetings, and required trainings
