Part-Time Position Details:
- Minimum availability of 15-20 hours per week (morning flexibility required).
- Maintain a minimum caseload of 4-15 members, to be built within six months or less at the manager’s discretion.
- Caseloads are determined in collaboration with the Support Coordinator Supervisor and will not exceed 24 members.
- Opportunity to transition to full-time based on performance and availability.
Support Coordinator Responsibilities:
Adhere to all responsibilities required by the NJ Division of Developmental Disabilities (DDD), including:
Support Coordinator Responsibilities
Support Coordinators at 7TH & Madison Extensions of Empowerment are responsible for ensuring individuals with Intellectual and Developmental Disabilities (ID/DD) receive comprehensive and person-centered support in accordance with the NJ Division of Developmental Disabilities (DDD) guidelines.
Core Responsibilities:
Member & Family Guidance
- Explain the Fee-for-Service (FFS) individualized budget system for individuals with developmental disabilitiesin NJ.
- Assist families in understanding and accessing services available through the DDD system.
Service Plan Development & Implementation
- Collaborate with the participant and their family to create a Support & Service Plan that:
- Defines necessary services and associated costs.
- Ensures the plan aligns with the individual’s budget.
- Selects appropriate service providers for implementation.
- Monitor the plan’s effectiveness through:
- Monthly phone check-ins.
- Quarterly in-person visits.
- Annual home visits.
- Ongoing reports and case meetings.
- Revise service plans as needed to reflect changing goals and support needs.
Member Advocacy & Navigation
- Support individuals and families in navigating the complex social service system to ensure access to needed supports.
- Effectively listen, communicate, and provide compassionate support to individuals and their families.
- Advocate for appropriate services while collaborating with providers and stakeholders.
Compliance & Documentation
- Maintain accurate and timely documentation of all support activities in iRecord and internal systems.
- Ensure compliance with DDD regulations and 7TH & Madison policies.
- Respond to all requests for information within 48 hours from:
- Consumers & their families.
- Service providers.
- DDD officials.
- 7TH & Madison management.
Financial & Legal Considerations
- Gain a working knowledge of financial and legal topics affecting individuals with disabilities, including:
- Guardianship
- Special Needs Trusts
- Wills & estate planning
Technology & Communication
- Utilize technology efficiently for:
- Communicating with members and providers.
- Creating and updating service plans.
- Budgeting within the DDD system.
Professionalism & Organizational Representation
- Represent 7TH & Madison professionally at all times.
- Maintain the highest standards of integrity, respect, and confidentiality.
- Meet all monthly deliverables and deadlines as required.

State of New Jersey Requirements for Support Coordinators:
✔ Bachelor’s degree in Social Work, Psychology, Sociology, Counseling, Gerontology, Special Education, or Rehabilitation.
✔ One year of direct experience working with adults (18 ) with developmental disabilities (includes professional, volunteer, and family experience).
✔ Ability to pass fingerprinting, drug screening, and criminal background checks.
✔ Valid driver’s license and registered, insured personal vehicle for travel.
Required Qualifications:
✔ Previous or current experience as a Support Coordinator.
✔ New Jersey residency required.
