Please note: This is an onsite position, Monday- Friday with no weekend requirements. KEY RESPONSIBILITIES Discharge Planning: Ensure efficient and effective discharge planning and referral functions to maximize throughput and transition of care. Coordinate Care: Manage the care and services for selected patient populations across the health continuum. Optimize Resources: Promote effective utilization and monitoring of healthcare resources. Collaborate: Play a critical role within the interdisciplinary team to achieve opti mal patient-centered outcomes. Identify Barriers: Proactively identify and address barriers to treatment, health outcomes, and discharge planning, including biopsychosocial and high-risk factors. Bridge Gaps: Serve as an essential link between the patient, healthcare team, payer, and community. Support Transitions: Work diligently to ensure smooth and appropriate transitions of care for patients. EDUCATION REQUIREMENTS State of New York Registered Nurse (RN) license required and in good standing. Associate of Science in Nursing (ADN) from an accredited school of Nursing required. The University of Vermont Health Network is moving toward an all Bachelor of Science in Nursing (BSN) workforce. Bachelor of Science (BSN) preferred for this coordination of services position, however, will consider ADN with commitment of continuing education upon hire. OR Master's in social work (MSW) from an accredited school of Social Work required. LMSW preferred. OR Master's Degree from an accredited institution in a Health or Human Services field. Licensure preferred. OR Bachelor of Science or higher Degree from an accredited Physical Therapy program. Licensed or eligible to be licensed to practice Physical Therapy in the State of New York. OR Bachelor of Science or higher Degree from an accredited Occupational Therapy program. Licensed or eligible to be licensed to practice Occupational Therapy in the State of New York. In addition, case management accreditation by a nationally recognized accrediting body for case management (examples: CCM, ACM or ANCC certifications), preferred; or commitment to obtain upon hire.
