A BETTER LIFE RECOVERY LLC is seeking a skilled Clinical Case Manager I to join our team. This critical position plays a guiding force behind the client's progress in treatment at our organization.
Key Responsibilities
The successful candidate will oversee, manage and monitor the progress of each individual client, contributing to their treatment planning. They will be responsible for exercising independent discretion and judgment in the administration of their duties, ensuring each client has a proper continuum of care throughout their treatment episode and at discharge.
Qualifications
To perform this job successfully, the individual must possess a minimum of a Bachelor's degree in an appropriate field of study. A Master's degree is preferred. They must also have completed the ADP Selective Screening Services background check and any other state, federal or company required background/record check. Additionally, they must have a valid driver's license and be able to be insured by our auto insurance policy.
Knowledge, Skills, and Abilities
The ideal candidate will have excellent communication skills, both written and verbal. They must be able to operate standard office machines and equipment, including telephones, computers, copy machines, fax machines, calculators, scanners, and shredders. They must also be professional, maintain a positive attitude, and relationships with coworkers. Finally, they must excel at both written and verbal communications to convey essential information.
Responsibilities
Develop person-centered care plans, including prioritized problem lists, SNAP, client-reported goals, measurable and achievable objectives, and interventions. Identify and prioritize individual needs and problems. Communicate with aftercare programs and business development, facilitating aftercare coordination and coordination of care between programs. Lead and provide group therapy sessions utilizing Evidence Based Practices. Manage a caseload of up to 12 clients, with minimum of weekly aftercare planning sessions, prepare clinical documentation accurately and within agency time frames.
