Key Responsibilities

  • Provide community-based mental health services to individuals across the lifespan, including assessments, treatment planning, and intervention implementation.
  • Collaborate with clients, families, and multidisciplinary teams to create and deliver individualized care plans that emphasize empowerment, resilience, and personal growth.
  • Utilize evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, Positive Reinforcement Techniques, and skills rooted in CBT and DBT frameworks.
  • Offer behavioral interventions, including cognitive processing, group dynamics, and operant conditioning, tailored to client needs.
  • Coordinate care with external providers such as primary care physicians, psychiatrists, educators, and local organizations to ensure a holistic approach to client support.
  • Facilitate and lead group sessions related to psycho-social rehabilitation (PSR), based on client treatment goals.
  • Maintain accurate and timely documentation of client interactions, progress, and treatment outcomes, adhering to organizational policies and legal requirements.
  • Participate in professional development opportunities, staying current on best practices in mental health care and evidence-based techniques.
  • Actively contribute to quality improvement initiatives through data collection, reporting, and collaborative efforts.
  • Develop and maintain strong relationships with community stakeholders, including families, educators, and other key influencers in clients' support systems.
  • Respond effectively to client crises, ensuring appropriate resources and interventions are in place.

For ACT/ACT TAY/IMPART teams: Demonstrate understanding of the Assertive Community Treatment model, including its core processes, interdisciplinary collaboration, and service delivery.

Qualifications

Education and Experience:

  • Bachelor’s degree in Psychology, Social Work, or a related human services field; or
  • Bachelor’s degree in an unrelated field with two years of relevant experience; or
  • Any combination of four years of higher education and related experience; or
  • Associate of Applied Science in Behavioral Health Support; or
  • Four years of qualifying experience delivering services to individuals with mental health disorders, substance use disorders, or developmental disabilities.

Qualifying Experience Includes:

  • Providing one-on-one or group services with a focus on recovery, rehabilitation, or resiliency.
  • Teaching and modeling coping strategies and management techniques for mental health, developmental, or substance use issues.
  • Supporting clients in achieving personal goals related to employment, education, and community engagement.
  • Assisting clients in implementing their individual treatment plans effectively.

Knowledge, Skills, and Abilities

  • In-depth understanding of case management and rehabilitation principles for mental health and/or substance use disorders.
  • Familiarity with community resources and the ability to connect clients to appropriate services.
  • Knowledge of evidence-based practices, legal requirements, and court procedures related to mental health and substance use treatment.
  • Strong interpersonal and communication skills, both verbal and written, with the ability to produce clear and concise documentation.
  • Self-motivated with the ability to balance multiple priorities and maintain a high standard of service.
  • Proven ability to establish effective relationships with clients, families, and professional teams.

Employment Requirements:

  • Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.

  • Completion of New Hire Orientation at the beginning of employment.

  • All training requirements including Relias at the beginning of employment and annually thereafter.

  • Current driver’s license, acceptable driving record and current auto insurance.

  • Must have reliable means of transportation in order to transport clients in personal vehicle.

Physical Requirements:

ADA Consideration - Sedentary work:  Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.  Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.

Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time.  Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
