Home Health Visiting Nurse (LVN only - 100% travel - Hemet, Winchester, and nearby areas)

Overview
We are seeking a compassionate and dedicated Home Health Visiting Nurse (with an active LVN license) to join our healthcare team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support and medical attention they need. The ideal candidate will possess a strong background in patient assessment and home health care, with the ability to work independently while maintaining effective communication with healthcare teams and families.

Duties

• Conduct thorough patient assessments to develop individualized care plans.
• Provide direct nursing care, including medication administration, wound care, and infection control measures as needed.
• Assist patients with daily living activities and educate them on health management practices.
• Collaborate with physicians and other healthcare professionals to coordinate patient care.
• Lift and assist patients safely during transfers.

Requirements

• Visit home health patients in Hemet, Winchester, and nearby areas
• Valid LVN license in the state of practice.
• At least 1 year experience in home health care
• Strong skills in patient assessment and critical thinking.
• Familiarity with Data Soft Logic or other electronic health record systems is preferred.
• Knowledge of infection control protocols and internal medicine practices.
• Ability to lift patients safely and effectively.
• Excellent communication skills, both verbal and written.
• Compassionate demeanor with a commitment to providing quality patient care.

Pay: Up to $45.00 per hour
