Education and Experience Required:
- A bachelor’s degree in Human Services, Social Work, Special Education, or other closely related Human Services area
- For AP, one (1) years of professional experience with the adult mental health population
- For QP. two (2) years of professional experience with the adult mental health population

Seeking a Qualified Professional to work with adults as a part of a Community Support Team. This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore an individual’s ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the individual’s community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal.
Job Duties include:
- Assists with the development of relapse prevention and disease management strategies
- Ensures linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations
- Provides intensive case management
- Completes functional needs assessment(s) to determine the scope and anticipated outcomes to the services
- Assist with the individual’s housing search including engaging landlords to rent to individuals and writing reasonable accommodation letters
- Assist with connecting individuals to financial and in-kind resources to set up and maintain their household
Basic Requirements:
- Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
- Valid driver’s license, auto insurance coverage and reliable transportation
- Pass criminal background check and other regulatory checks
- Provide proof of your education
