Key responsibilities may include :
Patient assessment :
Conducting comprehensive evaluations to assess patients' physical abilities, limitations, pain levels, and functional goals in their home environment.
Treatment plan development :
Creating individualized treatment plans based on patient needs, including specific exercises, manual therapy techniques, and modalities like heat or cold therapy, tailored to their home setting.
Therapeutic interventions :
Implementing physical therapy interventions such as strengthening exercises, range-of-motion exercises, balance training, gait training, and pain management techniques.
Patient education :
Instructing patients and their caregivers on proper exercise techniques, home safety measures, and self-management strategies to maintain progress after therapy sessions.
Collaboration with healthcare team :
Communicating with physicians, case managers, and other healthcare professionals to coordinate patient care and ensure treatment plans align with overall medical goals.
Documentation and progress monitoring :
Maintaining detailed patient records, including progress notes, treatment plans, and functional assessments to track patient progress and adapt treatment accordingly.
Home environment assessment :
Evaluating the patient's home environment to identify potential safety hazards and recommend modifications to facilitate optimal function.
End-of-life care focus :
Providing comfort-focused interventions in hospice settings, including pain management, positioning techniques, and supportive care to enhance quality of life during the final stages of illness.
Required skills and qualifications :
- Doctorate of Physical Therapy (DPT) degree
- Current state licensure as a physical therapist
- Experience in home health or hospice care preferred
- Excellent communication and interpersonal skills to interact with patients, caregivers, and healthcare team members
- Strong assessment and problem-solving abilities to develop individualized treatment plans
- Ability to work independently and manage a caseload in a home setting
- Knowledge of various physical therapy modalities and techniques
- Ability to adapt treatment plans based on changing patient needs and disease progression
