- RN-Case Manage (CHN)
Summary:
Provides and coordinates skilled home care services that reflect the mission, values, philosophy, policies/procedures and standards of care that A&D Health Care Professionals is committed to uphold. Provides and documents client care according to the client's plan of treatment. Communicates to and obtains pertinent information from the Clinical Supervisor/Branch Manager, other clinicians involved in a client's care, the physician and other agencies providing services to a client. Maintains confidentiality. Provides clinical direction and supervision for the Home Health Aide and LPNs providing care to the CHN's primary clients. Reports to the Director of Clinical Services/designee.
Qualifications:
- Graduate of an accredited School of Nursing.
- Currently licensed as a Registered Nurse eligible to practice in the State of Michigan.
- Minimum of one year experience as a Registered Nurse.
- Demonstrated knowledge of home health services and external regulations preferred.
- Positive, professional interpersonal skills.
- Meets CEU requirements necessary for re-licensure and health and malpractice requirements demanded of all clinical/field staff who provide service through the agency.
Essential Job Functions and Physical Requirements:
- Able to interact successfully with clients, family members, medical profession, long term care facilities, hospitals, other A&D Health Care Professionals employees, and case managers.
- Must be able to communicate both verbally and in writing with superiors, physicians, field staff, office personnel, and clients/families.
- Have visual/hearing ability sufficient to comprehend written/verbal communication and perform accurate physical assessments.
- Able to see, hear and use a telephone without the use of extraordinary assistive devices.
- Must be able to operate an automobile and have access to personal transportation.
- Able to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
- Must possess excellent customer relation skills.
- Must be able to complete multi-tasks and meet muti-deadlines.
- Able to work in a stressful environment.
- Has effective time management skills.
- Able to report for work on a regularly scheduled basis.
- Must be able to successfully provide effective team support for both office and field staff.
- Able to lift/transfer/pull up to 80-150 pounds frequently and up to 125-250 pounds infrequently using assistive techniques and devices appropriate to manage the task.
- Must be available for rotating "on call" weekends as needed.
