Job Summary
The Care Manager plays a crucial role in supporting individuals and families by providing comprehensive care management services. Responsibilities
- Conduct thorough assessments of clients’ needs and develop individualized care plans.
- Coordinate with healthcare providers, social workers, and community resources to facilitate access to services.
- Provide crisis intervention and support to clients experiencing challenging situations.
- Utilize motivational interviewing techniques to encourage client engagement and adherence to care plans.
- Monitor client progress and adjust care plans as necessary to meet evolving needs.
- Offer guidance in navigating Medicare and other relevant programs.
- Support clients in behavioral health challenges, child welfare issues, and addiction counseling as needed.
- Mentor and supervise junior staff or interns in best practices for client care management.
- Maintain accurate documentation of client interactions, progress notes, and service referrals.
Experience
- A background in social work or a related field is preferred.
- Experience with crisis intervention techniques and motivational interviewing is highly desirable.
- Familiarity with Medicare regulations and resources is beneficial.
- Knowledge of behavioral health practices, child welfare systems, Section 8 housing assistance, and addiction counseling is a plus.
- Proven ability to mentor or supervise staff in a professional setting will be advantageous.
- Strong communication skills, both verbal and written, are essential for effective client interaction.
