*Job Summary* As a Care Manager, you will be responsible for overseeing the care and support services provided to individuals in need. Your role involves coordinating and managing various aspects of care to ensure the well-being of clients. *Duties* - Utilize motivational interviewing techniques to engage clients and promote positive behavioral changes - Provide addiction counseling and support to individuals struggling with substance abuse - Coordinate services related to Medicare benefits and assist clients in navigating the healthcare system - Supervise and mentor staff members involved in delivering care services - Conduct crisis intervention and management when necessary to ensure client safety - Collaborate with agencies involved in child welfare and Section 8 housing programs - Address behavioral health concerns and develop appropriate care plans for clients *Experience* - Previous experience in crisis intervention, crisis management, and behavioral health settings - Proficiency in mentoring, supervising, and providing support to individuals in need - Knowledge of Medicare regulations and processes related to healthcare services - Familiarity with addiction counseling techniques and resources for individuals with substance use disorders
