- Strong Advocate: Championing for those experiencing barriers to health and well-being
- Patient & Wise: Exhibiting kindness, setting boundaries, and demonstrating sound judgment in assisting vulnerable clients
- Responsible & Reliable: A loyal team player committed to supporting the most marginalized, even in challenging situations
Key Responsibilities
- Immediate Support: Respond to urgent needs and, with the client, plan for greater stability
- Client Partnership: Work with clients facing significant health and well-being barriers to achieve greater stability
- Resource Navigation: Leverage quality information about community resources, community-based and on-site partnerships, and financial assistance programs
- Skilled Advocacy: Employ effective listening and a strengths-based perspective to advocate for vulnerable clients
- Team Collaboration: Engage in case-conferencing and teamwork for more comprehensive client care
