Bilingual Intake Case Manager

Purpose of Position- As part of our intake team, you'll be the first point of contact for individuals with disabilities in Boulder, Broomfield, and Gilpin Counties seeking community-based support and services. In this role, you'll manage a caseload of individuals with diverse needs and goals. Your responsibilities will include working closely with individuals and their caregivers to find the best program fit and ensuring all eligibility requirements are met. This may involve conducting assessments, coordinating funding requests, providing education on rights and responsibilities, and connecting individuals with needed resources. Performs all duties in compliance with applicable rules and regulations. 

Essential Duties/Responsibilities Responsible for completing various steps necessary for clients to access services.  

  • Taking responsibility for managing the enrollment process for individuals on their caseload, including independently tracking timelines, ensuring compliance with state rules and regulations, and organizing client files to facilitate ongoing case management. 

  • Ensures members are making an informed choice, by educating them and their caregivers on their program options, and relevant intake and enrollment processes. 

  • Conducts screenings and assessments to determine what support options a client may be eligible for and what programs or supports would benefit them. 

  • Reviews evaluations and documentation to determine if clients meet state criteria for Intellectual or Development Disability or Developmental Delay.  

  • Ensures clients health information is protected in accordance with HIPAA privacy rules; including collecting Release of Information forms when needed. 

  • Coordinates with outside agencies and sends referrals to connect clients with additional support if needed.  

  • Facilitates communication among clients, families, agencies, and support team members regarding enrollment updates and next steps. 

  • Participates in regular training sessions and stays updated on pertinent federal and state regulations, as well as A&I Avenues' policies and procedures 

  • Ensures timely completion and submission of enrollment documentation to appropriate agencies. 

  • Assists clients and families in resolving complaints, filing grievances, and providing support with formal appeals.   

  • Completes transitions and terminations as needed, ensuring proper notice of eligibility and discharge is provided.  

  • Connects clients with crisis intervention resources as necessary. 

  • Collects all necessary documentation and client signatures in compliance with applicable Federal and State regulations and A&I Avenues policies. 

  • Maintains accurate, professional documentation within established timelines.  

  • Completes any necessary follow-up to critical incidents and investigative reports.  

  • Provide case coverage in times of system change or staffing shortages. 

  • Supporting our bilingual team in translating documents for our Spanish speaking members.  

  • This position requires regular use of a personal vehicle with liability insurance that meets A&I Avenue standards. 

  • This position requires regular use of a personal mobile device such as a smartphone or tablet.  

  • This position requires fluency in Spanish. 

Other Duties/Responsibilities  

  • If position occupant has M.S.W., may supervise or assist in supervision of social work interns. 

  • Other duties as assigned.  

 

Knowledge, Skill, and Ability: 

  • Solid skills in working with people with disabilities. 

  • Independent judgment and discretion in monitoring, advising, and advocating for clients and their families. 

  • Person-centered, creative problem solving and strengths-based solution-finding. 

  • Ability to manage workload independently and prioritize tasks/goals in order to meet timelines. 

  • Ability to work effectively in a team setting.  

  • Effective and professional written and verbal communication skills with clients and groups at all professional levels.  

  • Strong planning, time management, and organizational skills.  

  • Ability to work efficiently with various technology including word processing software, e-mail, databases, and general internet skills.  

  • Possession of a valid driver’s license and ability to meet A&I Avenues driving requirements.  

          Training/Education: 

A person who has at least a Bachelor’s Degree and a minimum of two (2) years’ experience in the field of developmental disabilities or a person with at least five (5) years of experience in the field of developmental disabilities with competency in the following areas: 

  1. Understanding of civil, legal and human rights; 

  1. Understanding of the theory and practice of positive and non-aversive behavioral intervention strategies; 

  1. Understanding of the theory and practice of non-violent crisis and behavioral intervention strategies.   

 

          Working Environment/Physical Activities 

  • Effective with shifting roles, responsibilities, and expectations in a highly stressful and changing work environment.   

  • In the field workdays are required regularly.  Living arrangements on the Colorado front range strongly preferred. 

  • Ability to work autonomously while remote.
