Purpose of Position
A Systems Navigator provides specialized systems navigation to Boulder County residents who are in the process of eligibility determination. Individuals must be presumed to have an IDD, a Brain Injury or Autism to be eligible to work with the Systems Navigator. Works in collaboration with individuals, their guardians, and public benefits. Establishes and gives agency-developed assessment to determine whether a member is motivated to follow through with the enrollment process; to include barriers for success.
Provides temporary support through the duration of the eligibility process to ensure steps are being followed to obtain appropriate benefits. Provides resources and funding to support individuals with obtaining testing to identify an IDD, Brain Injury or Autism. Provides significant support to apply for public benefits such as Medicaid, Medicare, Social Security, and/or a trust fund.
Essential Duties/Responsibilities-
- Collaborates with local advocacy agencies, public assistance programs, service providers and when necessary, child/adult protection services, and law enforcement.
- Establishes and maintains expertise in various types of Medicaid, Medicare, SSI, SSDI, Trusts, TANF, subsidized programs, DVR, guardianship and alternatives, transportation, housing and other public benefits including Medicaid through Boulder County Human Services.
- Establishes and maintains relationships with local SSA offices. Navigates individuals’ challenges with SSDI/SSA and assists in solution development and implementation.
- Supports and encourages full inclusion in the community (including recreation, spiritual, healthy relationships (friends, romantic partners, interest groups, family, peers, colleagues)
- Communicates effectively in a culturally competent manner (both verbal and written)
- Demonstrated commitment to equity, inclusion and diversity
- Uses a person-centered approach to create an action plan to meet the individual’s goals
- Meets with individuals and/or families in person or virtual means (preference of individual)
- Identifies community systems barriers for the individual and/or family to access resources such as other available community resources and natural supports.
- Provides short term, time limited connections to resources for individuals and/or families
- Is dedicated to helping individuals and/or families develop comprehensive solutions
- Empowers individuals to self-navigate available resources
- Follows through to ensure goals are being met
- Problem-solving and coaching for how to navigate systems
- Will collect data to influence best practice systems improvement
- This position requires regular use of a personal mobile device such as a smartphone or tablet.
- This position requires regular use of a personal vehicle with liability insurance that meets Imagine! standards.
