Position Summary:
The Home Health Care Aide/Direct Care Staff are responsible for maintaining a safe, supportive, and therapeutic environment, which fosters growth and rehabilitation and facilitates the development of mutual aid/self-help skills among individuals. The staff’s primary role is to provide the appropriate supports tailored to the needs and desires of each individual, in order to enable each resident to live as independently as possible. The Home Health Aide/Direct Care Staff are expected to recognize when an individual is in a crisis and be able to utilize the appropriate psychiatric and medical back up services. This position is also referred to as a Direct Support Professional.
Job Requirements/Duties:
- Assist/monitor/teach individuals with preparation of daytime activities; encourage good personal hygiene; and behavior appropriate within the home
- Assist Program Specialist in the assessment of each individual(s) abilities, interests, and need areas toward independent living. Assist Program Specialist in the development of each ISP. Staff behavior and interaction with individuals shall be directed towards the attainment of goals set forth within each ISP
- Assist/monitor/teach individual(s) in budgeting skills
- Assist/monitor/teach individual(s) engaging in chores( i.e. making their bed, laundry, washing their dishes, etc.)
- Supervise and assist residents in punctual attendance of scheduled daily programs: i.e. school, church, therapy sessions, work, etc.
- Assist/monitor/teach individual(s) as needed in grocery shopping, menu planning, meal preparation and clean up
- Meet with individual(s) (as needed) to discuss concerns, issues, problems, etc.
- Assist/monitor/teach individuals in holding house meetings and informal discussion groups with peers
- Handle emergency situations appropriately - Utilize crisis interventions when necessary
- Teach and/or assist individuals in utilizing public transportation - Transport individuals to appropriate appointments only when necessary (i.e. person requires assistance at appointment)
- Perform daily record keeping(i.e. data collection/daily logs), and exchange information with next shift support staff
- Teach and be supportive of basic nutrition
- Teach relationship building skills
- Teach community integration; assist individuals in joining community organizations and groups of their choice
- Teach problem solving skills and conflict resolution
- Provide leisure/recreational activities of individual(s) choice and encourage individual(s) self- initiative in leisure activities
- Teach/monitor individual(s) self management skills (in health and behavior management)
- Teach individual(s) basic first aid, fire safety, emergency skills, sex education; and/or assist individual locating a class(community resource) in the above stated items
- Provide opportunities for individuals to have choices, make decisions in daily living, and their future goals (i.e. assist individual in day to day decisions)
- Assist in creating natural supports for individuals
- Assist in building individual(s) self esteem
- Encourage and promote empowerment
- Follow individual treatment plans consistently
- Attend individual and inter-agency staff meetings related to individuals
- Attend training courses/seminars
- Implement OSHA regulations
- Maintain the cleanliness and neatness of indoor and outdoor areas. Note: First and second shifts primary functions are based on consumer interaction. Third shift responsibilities focus on monitoring safety and completion of all household chores not completed by first and second shifts. These activities should occur primarily during asleep hours and should be completed before the end of shift.
- Perform antiseptic cleaning duties, laundry, and other housekeeping duties in living areas
- May work evenings, weekends, and holidays as needed
- Lifting, shoveling snow, and climbing stairs
- Perform other duties as assigned

Job Types: Full-time, Part-time
Pay: $15.00 - $16.00 per hour
