Job Title: Home Health Registered Nurse

Job Overview:
A-Line Staffing is currently hiring for a Home Health Registered Nurse role for a client in Lancaster County, SC. The Registered Nurse will provide skilled nursing care for patients in their homes, coordinating care with the interdisciplinary team and patient/family. This is a full-time position with guaranteed hours and the potential for long-term placement.

Home Health Registered Nurse Compensation

• Pay: $51 per hour
• Benefits are available to full-time employees after 90 days of employment.
• 401(k) with company match is available for full-time employees with 1 year of service on eligibility dates.

Home Health Registered Nurse Highlights

• This is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs.
• Required availability for on-call duties: 1 weekend per month and 1 night per week.
• Guaranteed 32 hours per week.

Home Health Registered Nurse Responsibilities

• Administer skilled nursing care for patients of all ages in their home.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC).
• Educate patients and families regarding care, diet, medications, and home safety.
• Supervise and provide clinical direction to HHA/STNA/LVN for quality service delivery.
• Perform on-call duties as required.
• Attend interdisciplinary team meetings and ensure communication with all stakeholders.

Home Health Registered Nurse Requirements

• 1-2 years experience as a Registered Nurse in a clinical care setting required.
• 2 years of home health experience required.
• Current state license as a Registered Nurse in South Carolina.
• Experience with Homecare Homebase Software and OASIS required.
• Current CPR certification required.
• CHPN certification preferred.
• Reliable transportation and a valid driver’s license are required.
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program required; BSN preferred.

Home Health Registered Nurse Preferred Qualifications

• CHPN certification is preferred.
• Bachelor’s of Science in Nursing preferred.
