Long-Term Care (LTC) Pharmacist Compensation
- The pay for this position is $61.00 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Long-Term Care (LTC) Pharmacist Highlights
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- The required availability for this position is:
- One shift: 8:00 AM – 4:30 PM
- One shift: 11:00 AM – 7:30 PM
Long-Term Care (LTC) Pharmacist Responsibilities
- Evaluate physician medication orders for drug appropriateness, dosage, potential interactions, and route of administration
- Make therapy recommendations to physicians and facility staff as needed
- Maintain patient database, including medical history, lab data, and concurrent medications
- Ensure laboratory tests for monitoring high-risk conditions are appropriately ordered and reviewed
- Collaborate with the healthcare team to provide clinical and technical support
- Document all communication with physicians, nurses, healthcare team members, and patients in medical records
- Implement policies and procedures for the safe procurement, storage, distribution, and disposal of medications
- Ensure compliance with federal and state regulations and company policies
- Report medication-related errors to the Pharmacy Manager
- Assist facility staff with procedures for safe medication procurement, administration, and disposal
- Perform other related duties as assigned
Long-Term Care (LTC) Pharmacist Requirements
- Current New Hampshire Pharmacist License (Required)
- Bachelor’s Degree in Pharmacy (PharmD preferred)
- Knowledge of pharmacy laws and federal/state regulations for long-term care
- Ability to work collaboratively as part of a team
- One year of experience in long-term care preferred
