Care Plan Reviewer (Clinical Case Manager - Behavioral Health)
Pay: $27.28 per hour (Biweekly Pay)
Schedule: Monday to Friday, 8:00 AM – 5:00 PM (No evenings, weekends, or holidays)
Work Environment: Hybrid (Primarily remote with up to 10% travel to Columbus, OH)

Your Responsibilities Will Include:
Member Assessment & Care Planning:
- Conduct detailed assessments using clinical tools to evaluate members' needs and eligibility.
- Develop and implement tailored care plans addressing physical and behavioral health needs.
- Use clinical judgment to resolve complex cases, reduce risk factors, and provide crisis intervention.
Care Coordination:
- Collaborate with healthcare providers to ensure seamless member care.
- Connect members to psychosocial wraparound services and community resources.
- Present complex cases in multidisciplinary conferences to achieve optimal outcomes.
Quality Assurance:
- Apply clinical guidelines and case management policies to ensure appropriate benefit administration.
- Document actions, outcomes, and member progress in compliance with regulatory standards.
What You Bring:
Education & Licensure:
- Active and unrestricted Behavioral Health clinical license in Ohio (e.g., LMFT, LIMFT, LSW, LISW, LPC, LPCC).
- High school diploma or GED (required).
Experience:
- 3 years of post-master’s direct clinical practice experience in hospital or outpatient settings.
- 2 years of community-based services experience with children, youth, and families (mental health, child welfare, or developmental disabilities).
- Familiarity with family systems, case management, child therapy, and child protection (preferred).
Skills:
- Proficiency with MS Office Suite (Excel, Outlook, Word) and multi-system navigation.
- Strong communication, organizational, and problem-solving skills.
Additional Requirements:
- Ohio residency and reliable transportation for occasional travel (mileage reimbursed).
- Comfortable traveling up to 10% for professional meetings and training sessions.
