Care Manager (RN) Compensation
• The pay for this position is up to $42 - $45 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Care Manager (RN) Highlights
• The required availability for this position is Monday – Friday, 8:00 AM – 5:00 PM
• This is a remote position, but candidates must be located in the Dayton, OH area
Care Manager (RN) Responsibilities
• Conduct face-to-face assessments and visits with members in the field
• Manage caseloads for both the over-60 waiver population or the under-60 population
• Ensure the safety and welfare of members through in-person visits and audits
• Work with an EMR system to prioritize daily tasks and maintain accurate documentation
• Collaborate with the utilization management and waiver service coordination teams
Care Manager (RN) Requirements
• Active RN license in Ohio (Compact License accepted)
• Associate’s degree in Nursing required
• Home Health experience required
• Strong technology skills, including proficiency in multiple systems
• Excellent time management and prioritization skills
• Must be a self-starter and able to navigate a constantly changing environment
• Attendance is mandatory for the first 90 days
Care Manager (RN) Preferences
• Waiver knowledge and experience preferred
