Position Summary
As a Social Worker/RN Case Manager, you will:
- Manage a continuum of care for high-risk patients, supporting them toward better health outcomes and self-sufficiency.
- Conduct assessments, create service plans, and coordinate care with community and healthcare providers.
- Participate in utilization review, ensuring medical necessity of referrals, and reducing care fragmentation.
- Work in close collaboration with providers, PRC, and the Director of Case Management.
Education & License:
- RN: Associate’s Degree in Nursing required; BSN must be completed within 2 years of hire
- BLS Certification (American Heart Association) – required and maintained throughout employment
- Unrestricted RN License (any U.S. state, territory, or commonwealth)
- Minimum of 3 years in a clinical care setting
- 1 year of travel experience required
- Familiarity with case management, discharge planning, and utilization review
- Experience using electronic health record systems
