Senior LTSS Service Care Manager Compensation

  • The pay for this position is $60 per hour .
  • Mileage is reimbursed.
  • Benefits are available to full-time employees after 90 days of employment.
  • A 401(k) with a company match is available for full-time employees with one year of service on our eligibility dates.

Senior LTSS Service Care Manager Highlights

  • This is a 100% field position .
  • Work hours are 8 : 30 AM to 5 : 00 PM, Monday through Friday .
  • The role requires 23 visits per day , with each visit lasting 23 hours .
  • Travel to members homes is required, with no remote or work-from-home option.

    • Senior LTSS Service Care Manager Responsibilities

  • Conduct home visits to assess and provide care management for members.
  • Prepare for visits by reviewing member records and previous assessments.
  • Utilize critical thinking and problem-solving skills during visits.
  • Document visits in real time and ensure accuracy using the provided tools.
  • Use technology, including Lenavi, Word, Excel, and other software.
  • Navigate various weather and home environments with a non-judgmental attitude.

    • Senior LTSS Service Care Manager Requirements

  • Active Registered Nurse (RN) license in New York.
  • UAS (Uniform Assessment System) experience preferred.
  • Valid NY State Drivers License and reliable transportation.
  • Ability to work independently in the field and manage a flexible schedule.
  • Proficiency in Microsoft Office applications (Word, Outlook, Excel).
  • Mandatory attendance for the first 90 days .

    • Senior LTSS Service Care Manager Preferred Qualifications

  • Experience with MLTC (Managed Long-Term Care), MAP, or Maximus .
  • Bilingual in Spanish is a plus.
  • Bachelor of Science in Nursing (BSN) preferred.
