Field Case Management Coordinator
Location: Rockford, IL
Payrate: $40.86
Work Hours: Monday - Friday, 8:00 AM - 5:00 PM
Employment Type: Full-Time, Remote 25%Travel
We are seeking a dedicated and compassionate Field Case Management Coordinator to provide essential care coordination for elderly individuals and those with disabilities in the McHenry/Winnebago County areas. The role will involve traveling to members' homes and working remotely in between visits. The ideal candidate is organized, efficient, and able to work independently.
Key Responsibilities:
- Evaluation of Members:
- Conduct comprehensive evaluations of referred members' needs, eligibility, and benefit plans.
- Recommend care approaches and assist with identifying high-risk factors and service needs.
- Collaborate with clinical case management and crisis intervention when necessary.
- Enhancement of Medical Appropriateness and Quality of Care:
- Consult with case managers, supervisors, Medical Directors, and other health programs to address barriers to care.
- Present cases at case conferences to ensure multidisciplinary review for optimal outcomes.
- Utilize negotiation and motivational interviewing skills to secure necessary services and engage members in their care.
- Empower members to make informed decisions and take an active role in managing their health and lifestyle.
- Monitoring, Evaluation, and Documentation of Care:
- Ensure all case management and quality management processes align with regulatory, accreditation, and company standards.
- Document and monitor care plans in compliance with policies and procedures.
Qualifications:
- Required:
- Bachelor’s degree in Behavioral Health or Human Services (psychology, social work, marriage and family therapy, nursing, counseling, etc.) or a non-licensed master's level clinician.
- Must be a resident of McHenry/Winnebago Counties, IL due to travel requirements.
- Ability to work independently and demonstrate excellent organizational skills.
- Preferred:
- 2 years of experience in case management or a related field (e.g., behavioral health, social services).
- Previous experience in managed care or similar healthcare settings.
- Prior experience with care coordination or health assessments.
