Responsibilities:
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine clinical diagnosis and treatment plans.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Receive and carry out physician orders.
- Notify the patient physician, RN, and/or Clinical Supervisor immediately of any changes in the patient’s condition.
- Include the patient and the family in the planning process.
- Document direct patient services, ensuring thorough coordination and collaboration with physicians and other disciplines
- Current California LVN license.
- Driver license, Auto insurance
- Valid CPR card
- Ability to travel to the prospective patient's place of residence
