What are the responsibilities and job description for the Care Coordinator in Youth Shelter position at A New Day Youth and Family Services?
Overview
Provide care coordination for all young people in the Safe Home which include
coordinating necessary services and resources with formal and informal supports; identifying and supporting access to safe and stable housing options; building bridges of support and communication between young people and their formal and informal support system.
Care Coordination
· Develop individualized service plans for each youth based on their assessment, ensuring that their needs, goals, and safety are prioritized.
· Support, engage, and empower young people in developing and meeting their goals
· Develop the Shelter Plan (updated every 30-days), Education Plan and Safety Plan within 72 hours and complete the ACEs Questionnaire, CSE-IT assessment, and Greatness Surveys within 7 days of the young person entering the Safe Home.
· Hold bi-weekly team meetings to support discharge planning and partnership between all entities
· Enroll youth in school, track and document all related educational progress. Support YES staff in developing educational materials. Act as the educational point of contact for all youth in the SafeHome
· Assist with coordinating and scheduling necessary medical appointments and coordinate with teams/families to ensure all current medications are available in the safe home for youth.
· Maintain meticulous records, including Daily Case Notes for all young people, and interactions related
to guardians or other members of the young person’s family or team
· Act as the primary point of contact for coordinating services
· Transport young people to necessary appointments and to access needed resources
Minimum staff qualifications are:
· Shall hold a bachelor’s degree or above in social work, counseling, psychology or a related field, from an accredited university; or
· shall hold a bachelor’s degree or above in another field with two (2) years relevant experience working with the target population; or
· shall hold an associate degree from an accredited college and have a minimum of three (3) years of experienceworking with the target population; or
· shall be a high school graduate or have a general education development (GED) and shall and have a minimum of three (3) years of experience working with the target population.
· Must be 21 years of age
Job Type: Full-time
Pay: $18.00 - $20.25 per hour
